Integrated Behavioral Health

At Appalachian Mountain Community Health Centers (AMCHC), behavioral health is integrated into the primary care atmosphere. We consider the whole person when stress, anxiety, adverse behaviors, emotional concerns, or other life circumstances are interfering with your daily life and overall health. You’ll work with a Licensed Behavioral Health Clinician and your primary care provider in tandem to evaluate the mind-body-behavior connection in order to provide solution-focused care and interventions. 

Integrated Behavioral Health Services are available to all patients of Appalachian Mountain Community Health Centers. There may be a copay for individuals with private insurance or Medicare.

WE CAN ADDRESS YOUR CONCERNS

  • Chronic medical conditions such as headaches, sleep problems, high blood pressure, asthma, diabetes, obesity, or chronic pain.

  • Emotional or behavioral health issues such as depression, anxiety, substance abuse, traumatic stress, or bereavement.

  • Behavior change plans for smoking cessation, weight loss, substance use, exercise, and lifestyle modifications.

The services provided by your Behavioral Health Clinician are simply another part of your overall primary health care and are not specialty mental health care. Documentation of your appointment and recommendations from the Behavioral Health Clinician will be written in your medical record, accessible by your primary care provider.

Upon your request, or if the Behavioral Health Clinician thinks you would benefit from specialty mental health services, they will assist your primary care provider with referring you to the services you need. 

 

WHAT TO EXPECT

Your discussion with the Behavioral Health Clinicians will occur at your AMCHC primary care location and take approximately 15-30 minutes. The BHC will ask you specific questions about your physical symptoms, any emotional concerns you are experiencing, your behaviors, and how all of these might be related. 

Your Behavioral Health Clinician and primary care provider have a close working relationship. While you and your primary care provider remain in control of your health care, Behavioral Health Clinician serves as a resource to assist you and your provider in developing and implementing the best whole person, integrated health care plan for you.
